Before your intake appointment, please do the following:

1. Read these five documents (I recommend that you print these for your records):

• Informed Consent for Psychological Services
• Informed Consent for Telehealth
• Financial Policies
• Louisiana Privacy Notice Form
• Right to Receive a Good Faith Estimate 

2.  I will email you links to the following three forms, to be completed and submitted via secure email:

• Registration Form
• Psychological Services Agreement
• Adult Therapy Questionnaire
